  Transcript Request  

How to Request a Transcript

To request a transcript, students must come into the Registrar's office and fill out a written request. If you are unable to come in person, you can put your request in writing. There is a fee of $3 for the first transcript and $1 thereafter. Please allow one week for your request to be processed. Checks may be made payable to Hart District. Written requests should include:

· Name while in school
· Date of birth
· Year graduated
· Where you would like the transcript mailed
· Phone number where you can be reached
· Fee
· Signature
· Copy of picture ID
* Only students and institutions may request copies of transcripts*
