Free COVID-19 Vaccination Clinic

2 pm - 8 pm
Participants will receive their first dose of the Pfizer vaccine
Families and students 16-and-over are welcome to sign up

On the day of clinic, please bring:

Photo ID (You do not need to show government issued ID and you do not need to be a US citizen to get a vaccine.)

Copy of insurance card (COVID-19 vaccines are free regardless of insurance status)

Completed consent/release form (parent/guardian's signature is required for students under 18).
