Free COVID-19 Vaccination Clinic

Participants will receive their first dose of the Pfizer vaccine
Families and students 12-and-over are welcome to sign up

On the day of clinic, please bring:
- Proof of Age (i.e. photo ID, school transcript, etc) (You do not need to show government issued ID and you do not need to be a US citizen to get a vaccine.)
- Copy of insurance card (COVID-19 vaccines are free regardless of insurance status)
- Completed consent/release form (parent/guardian's signature is required for students under 18).
- An Adult MUST accompany a minor aged 12-15
Second dose appointment will be automatically booked for June 11, 2021 at Sierra Vista JHS
